Severe sore throat,swollen glands, and trouble swallowing? When is it time to see a doctor?

Wow,I have never been in so much pain in my life. This feels much worst than when I had strep throat in the past. Whenever I try to swallow, even my own saliva, I have a pain in my throat, and in both ears (similar to an ear infection). This has been getting steadily worst over the last four days. Also whenever I try to gargle with salt and water, it feels like it's not actually reaching my tonsils ( I know this sounds weird).
